Who is responsible
The controller of the data you send through this site is Panos S. Gartaganis, ophthalmic surgeon, 12 Plastira Street, Voula 166 73, Athens, Greece. For any question about your data, write to info@panosgartaganis.gr.
What is collected
What you type in the contact form: your name, email, phone number (if you give it), the subject and your message.
To limit spam, the site keeps the IP address you send the form from for 10 minutes, then deletes it. The legal basis for this is the legitimate interest in keeping the site secure.
Please do not send medical reports or detailed health information through the form. These are best discussed at your appointment.
Why
So that the doctor can reply to your message and, if you ask, book an appointment. The legal basis is your consent, which you give when you send the form. Your data is not used for advertising and is never sold.
How long
Panos S. Gartaganis keeps your data for as long as the correspondence with you requires. After that he deletes it.
Who else sees it
Messages pass through the providers that host this site and deliver its email. They process them only on his behalf and only to deliver the message.
Visit statistics, if you allow them, are processed by Google (Google Analytics) on his behalf. Google may transfer data to the United States under the EU-U.S. Data Privacy Framework.
Cookies and statistics
This site uses Google Analytics for visit statistics: which pages are read, on what kind of device, and where visitors come from. Google Analytics loads and sets cookies only if you click «Accept» on the cookie notice. The legal basis is your consent. The statistics are kept for 14 months.
The Contact page has a Google Maps map. The map loads only with your consent or when you click «Show map». Google then receives your IP address and may set its own cookies, under Google's privacy policy.
This site sets no advertising cookies. You can change or withdraw your consent at any time from «Cookie settings» at the bottom of every page. When you withdraw it, the site deletes the Google Analytics cookies.
Your rights
You have the right to access, correct, delete, restrict and port your data, and to object to the processing of your IP address. You can also withdraw your consent. To do so, write to info@panosgartaganis.gr.
You can also lodge a complaint with the Hellenic Data Protection Authority (www.dpa.gr).
